I think your usability study is solid. I read through it and I really like how the the usability study has an obvious flow that resembles the instructions. This will make it very straight forward for the person taking the usability test to follow along with the instructions, having questions that will help him give important feedback about each step of the instructions. Your questions under each step are very relevant and seem to be questions that users would probably be asking themselves. I don't feel like there is really anything you need to change. If you were to change something, you may add a little more information in the introductory paragraph to help others have a better feel for what's ahead, but I don't know how important this is.
I think your Usability Test looks good. I like the organization of your questions. I think it is very helpful how you have parts, a,b,c, etc to each number question. I think this will enable you to recieve a lot more detail from the user, as well as help to guide the user in completing the usability test. Additionally, I think your final paragraph of concerns is helpful because it allowed me to see some struggles that you were having, but also ensured me that you are searching for an effective solution. A couple things to note, is that your last three questions are in a different font than the first six. Additionally, it might be helpful to elaborate on your initial paragraph a little bit. Overall, I think it looks very good!

I believe this usability test will be more like the report when you were trying to set up instruction. It will serve as a guideline for you to set up instruction that will tailor to your clients knowledge
