Overall a very nice looking final usability test draft. I really like your header, with I would have done that. I also like the lines for a awnsers, that is a great touch. Uploading code is beyond my comprehension, but I think with the awnsers to this usability test you could make your instructions even better, so that even I could do it.

The format of your usability test is excellence. The
introduction paragraph gives the user an idea about the purpose of your
usability test. In my opinion the questions that you use tailor to your client’s
need. If I am the one who is going to use your instruction, I probably will ask
some of the question in your usability test. I like the way you put lines under
your questions. It will guide user to give their answer in appropriate length.
