I really liked your questions, with some being the same as mine, with others being completely different that I would've never thought of asking the user. For example, the question of the user's comfort in switching to a paperless environment using this application. That is a great question to ask in order to get feedback. Also, the way you numbered the scale/ranking so all the user needs to do is circle an answer is a great idea. For the multimedia presentation, if we are to use Adobe Captivate and make a flash usability study/test, we could use something along these lines. All in all, your individual usability study is good, and we should integrate all of ours soon.
