Overall I think you have a very good usability test here. Its hard for me too be too accurate without actually seeing your instructions but you are definatly on the right track. One thing to consider is the first several questions are all just yes and no. You may want them to ellaborate some. At the end of the question maybe ask if they did have trouble where they experienced it. Right now they can just answer no and have answered your question. With these tests the most important thing is to get as much feedback as possible. You did a good job on the last four questions on trying to get more feedback, but I would incorporate some of that into the other questions. Overall I think the questions are good and it will be a very useful usability test.
That sounds pretty good Josh. Since you are using a complex program it's very important to ensure that the client does fully understand how to use it. I do agree with the previous comment that you sohuld elaborate out the questions. Again you are working with a very difficult program and you have to make sure that your client has a very good working knowledge of it from your instructions. You should probably elaborate on the matricies section as discerning the difference between * and .* is a bit confusing for first time use.
