This is a very good individual usability test. I like how detailed you go into in your answers. The questions you have are excellent but some of them seem a little advanced. I know this will be for people who are technical and know what they are doing but if customers were to take this usability test they may be confused from the start. You may want to add an "Advanced" section where you can ask questions to users who may have experience with this application. Other than that I think this is an excellent usability test that focuses on important areas of the program.

I really like how you address places that really have a possibility to pose problems.  They may be a little advanced, but I'm sure that who ever is accepting your proposal and eventually carrying out the tasks will know what you are speaking of.  The answers you provided added a lot of inside feedback to work on.  You seem to have a good idea of what issues need to be addressed and/or explained in more detail in the presentation.
